The following REQUIRED MVHS documents must be printed, completed, and returned during Diablo Daze:
- Diablo Days Registration Instructions and Checklist
- Postcard (mailed home the week of July 19th)
- Signed Student Emergency Card (printed at the last step of Data Confirmation)
- Signed Annual Notification Signature Form (printed at the last step of Data Confirmation)
- MVHS School Dance Contract
- 2021-22 Student Handbook

The forms listed below are optional. Read & review each of the forms below. Print, complete, and return all of the forms that apply to your student during Diablo Days:
- Library Printing Flyer
- School Picture Order Flyer
- ASB Items for Sale
- Parking Permit Application (Juniors and Seniors Only)
- PTSO Membership and Volunteer Form
Finally:
You MUST return to the parent portal to finalize the data confirmation process. Visit the Parent Portal if your page has expired.
