Medical Conditions

SUPPORT FOR STUDENTS WITH MEDICAL NEEDS:

If a student has a medical condition, a serious illness or accident, the school should be aware. An Emergency Care Plan or written orders from your physician are needed so staff are aware of signs or symptoms of a problem and what steps to take:

SVUSD has written Care Plans available online, OR will accept written instructions or Orders signed by their physician. Please see the following sections for : EMERGENCY CARE PLANS, PRECAUTIONARY CARE PLANS, AND SPECIALIZED HEALTH CARE PLANS.

Procedures:

  1. Parents Notify the school during Registration.
  2. The School can print out copies of the SVUSD forms, or give parents the "step-by-step" instructions for printing the forms out for their doctor to complete.
  3. Parents return SIGNED COPIES of the Emergency Care Plan or Special Health Plan to the school.
  4. The Health Office will notify the school staff including: Teachers, Food Service and the Playground/Campus Supervisor sso they are aware of any symptoms to watch for and steps to take.
