Open Enrollment 2019

FOR ALL BENEFIT ELIGIBLE EMPLOYEES

SVUSD’s annual Open Enrollment period will run from 10/29/18 – 11/16/18.

 

For 2019, we will continue to offer all the same health plans, Blue Shield Access+ HMO*, Trio HMO*, and PPO, Optum Mental Health*, Dental Dental, VSP Vision, and Voya Supplemental Life Insurance.   Our Annual Open Enrollment period is your chance to make any changes to your benefit selections without experiencing a Qualifying Event.     

 

So what do you need to do?

 

  • If you are a PPO enrollee AND in the CSEA (Classified employees) or SVMTA (Management) groups – You will need to complete a new  to acknowledge the 2019 PPO differential.  For 2019, the PPO differential amount will be $258/tenthly in addition to the regular premium. 

 

  • For all others who want to maintain their current health benefits for 2019, you do NOT need to do anything during this Open Enrollment.  J

 

  • If you want to make a change (e.g., add/drop a dependent or change medical plans), you will need to submit a  and a .

     

    • If you are adding a new dependent, please note that you will be required to provide applicable eligibility documentation:

      • Spouse or Domestic Partner – government issued marriage certificate or Domestic Partnership Registration document

      • Children – government issued birth certificate, adoption document, guardianship document, or other court order document

      • You may also use the first page of last year’s tax return if it lists your dependent’s information.

 

  • For Blue Shield Trio HMO enrollees, Blue Shield will fund a Health Reimbursement Account (HRA) for 2019 based on your coverage tier – Employee Only $200, Employee + 1 Dependent $400, Employee + 2 or More Dependents/Family $600.

    • Please note that if you make any coverage tier changes during the year, your HRA amount may be impacted.

  • Please note that Flexible Spending Account (Medical and Dependent Day Care) enrollments do NOT automatically continue for 2019.  If you wish to be enrolled in the Medical and/or Dependent Day Care Flexible Spending Accounts, you must submit a .  Please note that medical FSA deductions will be taken over 10 pays beginning in 2019, instead of 7.

     

  • If you are interested in enrolling in the Supplemental Life Insurance through Voya, please contact the Benefits Department for the application.

 

PLEASE NOTE IF YOU HAVE TRUSTMARK OR AMERICAN FIDELITY POLICIES:

 

 

We have a temporary hold on any new enrollments into the individual voluntary Trustmark benefits (Accident, Cancer, Disability, and Universal Life).  If you are currently enrolled in a Trustmark policy, it remains in place with no change.  If you wish to cancel or decrease a Trustmark coverage, you can call Trustmark Customer Care at 800-918-8877 during our Open Enrollment period.

 

 

For those of you who have individual policies with American Fidelity, you may continue those policies as well.  If you wish to cancel, you will need to complete their .  Please note that they do not allow the signature date and the effective date to be in the same month.  The cancellation date (drop effective) should be the last day of the month in which you want coverage (e.g., 12/31/18)

 

ALL Open Enrollment forms must be turned into the Benefits Department no later than Friday, November 16, 2018 to be processed for Open Enrollment, with all changes taking effect January 1, 2019.
