The District has partnered with VOYA to provide eligible employees and retirees with Basic Life and Accidental Death and Dismemberment insurance. Eligible employees are not required to contribute toward the cost of Basic Life insurance for them and any covered dependents. Eligible employees are covered under this plan regardless of enrollment or declination of any of the District's medical benefits.
Dependent Coverage: Dependents under the age of six months are covered for $200. Dependents age six months to 26 years old are covered for $2,000. Dependents are only covered under this benefit if an enrollment form and proper verification are submitted.
Employee Coverage: Employee coverage is dependent upon age at death or dismemberment. Please see below for coverage amounts due to death. For coverage amounts due to dismemberment, please contact the Benefits Department.
- Age 29 and under: $150,000
- Age 30 through 34: $137,500
- Age 35 through 39: $125,500
- Age 40 through 44: $100,000
- Age 45 through 49: $87,500
- Age 50 through 54: $62,500
- Age 55 through 59: $43,750
- Age 60 through 64: $24,000
- Age 65 through 69: $15,500
- Age 70 through 74: $10,500
- Age 75 through 79: $7,000
- Age 80 and over: $5,000
Please be advised that only eligible employees who are in active status OR enrolled in the District's pre-65 retiree benefits at the time of death or dismemberment are eligible for this benefit. Individuals enrolled in Spouse as Primary, the Hartford, or no longer employed with SVUSD are not covered.
