School Sites: Facilities Project Approval Form
Schools can submit a Facilities Project Approval Form to the Facilities Department for review and approval for a project on their campus. Please complete the entire form, attach a requisition, as well as drawings, estimates and as much information as possible regarding the project.
Notify the Facilities Department if the work will be completed by volunteer adults, as this needs District approval as well. Once approved, an Adult Volunteer Form must be signed by each volunteer before the work begins at the school site. The form is available in the link above and is posted below on this webpage.
