Schools can submit a Facilities Project Approval Form to the Facilities Department for review and approval for a project on their campus. Please complete the entire form, attach a requisition, as well as drawings, estimates and as much information as possible regarding the project.

The Facilities Project Approval Form is listed below can also be found on the District website's All Forms page under Employee. Alternatively, the form can be obtained by contacting the Facilities Department.

Notify the Facilities Department if the work will be completed by volunteer adults, as this needs District approval as well. Once approved, an Adult Volunteer Form must be signed by each volunteer before the work begins at the school site. The form is available in the link above and is posted below on this webpage.
