Please follow the steps below to complete the Data Confirmation.

Step 1 - Parent Portal Account

If you already have an active Parent Portal Account, proceed to the next step. If you do not already have a Parent Portal Account, create an SVUSD Parent Portal Account through the SVUSD Family Portal. An SVUSD Parent Portal Account (not a Student Account) is required to register. (See Step 1 below.) All Data Confirmation paperwork will be completed online prior to the start of school.

NOTE: it is common for parents to use the Family Portal with a STUDENT account unknowingly, but they need to use their PARENT account instead of a student account for Data Confirmation.

How do I know if I have a Parent account in Family Portal? When logged into your Family Portal account, go to the Profile tab. Check the Linked Portal Account email area for the email and type of account. Type P is Parent account and Type S is a student account.

Step 2 - Data Confirmation (opens in July)

Complete DATA CONFIRMATION. Go to the AERIES Family Portal. Follow all prompts and tabs within the Data Confirmation pages to confirm or correct your student's data. (Use Helpful Screenshot Instructions if needed.) In the Data Confirmation window, you will need to do the following:

1. Review and update demographic and emergency contacts and medical information
2. Print and sign the required forms. This includes the Annual Notice to Parents, or ANTP.
3. Return to parent portal to complete the data confirmation process

Step 3 - Return Forms

All signed and emailed forms for data confirmation are due by mid-September of 2024. Exact date determined by our district is forthcoming.

• School office opens at 8:00 AM
• Parking lot is for "drop off" only. All parking lot spots are assigned for teachers and staff.
• School maps are available in the office for your convenience.
• Login to your Family Portal to find your student's teacher.

The following information is for the 2024-25 school year. Information about 2024-25 enrollment will be available in spring of 2024.

If you are a resident of de Portola Elementary School, online registration can be done at home through AIR.  Please click on the link to begin registration: Aeries AIR Page.

After completing the online registration, please proceed with your student(s) enrollment by bringing the necessary documents to de Portola's office.  We will begin processing the 1st through 6th grade documents in April of 2024.

Complete registration and enrollment will ensure class placement. Incomplete student registration/enrollment will not be processed until complete.

Necessary documents to submit and complete the student(s) enrollment are:

• Birth Certificate or Current Passport
• "Up-to-date" Immunizations
• TWO proofs of residency - utility bills ONLY (gas, water or electric)
• Release of student records (1st through 6th graders only)

Please bring all documents to the school to complete your student's enrollment.

If you have already completed online registration for your SVUSD student or if you have already attended another SVUSD school, you will not need to re-register online.  Your student is already in our system and enrollment documents are the only things needed.  Please come to our office to turn in paperwork.

All immunization records must be entered into AERIES (online registration) and completed prior to enrollment. Please review the state mandated “Parents’ Guide to Immunizations Required for School Entry”.

A copy of immunization records need to be given to our office.

We are unable to complete registration and enrollment of students for who are not in compliance with CA Department of Public Health.  If your child is not in compliance with all immunization requirements, you are urged to contact your family physician as soon as possible, as it may require time to obtain these immunizations. State Law "requires that children be up to date on their immunizations (shots) to attend school."

If your child is "missing" any immunizations, please be sure these are updated and in compliance prior to the first day of school, August 14, 2024.

Completed on-line AERIES registration- please print and sign the school copy.

Official birth certificate or passport

2 proofs of residency: Current utility bills - gas, water, or electric ONLY. Mobile/phone bills nor trash bills can prove residency.

Release of student records: Parents release of records for students that are coming to de Portola from a different school.  (Only necessary for 1st through 6th graders, form provided below.)

Up to date immunizations records - yellow card or official doctor's record will be needed.

1st Grade Physicals are required to prior to first grade entry (CA State requirement) - form required with doctor's stamp and signature.  Please contact our Health Office Aide (jenelle.glatfelter@svusd.org) with any questions relating to immunizations or first grade physicals.
