California law guarantees certain benefits to employees who are injured or become ill because of their jobs. All work related injuries or illnesses must be reported directly to Company Nurse at 888-770-0929 immediately, as well as to the direct supervisor.

Employees and volunteers are eligible to pre-designate their personal physician to be their immediate treater in case of a work related injury or illness. In order to utilize a personal physician, the employee and physician must both complete and sign the pre-designation form below and return it to the benefits department prior to an injury occurring. A pre-designation can be completed at any time, but will only be in effect for all subsequent injuries.

If an employee does not have a pre-designation on file, they may seek treatment within the District's medical provider network (MPN) at no cost. Below is a list of facilities within the network employees may treat at. Please be advised that any treatment outside of the below facilities or with a physician that has not already been pre-designated is considered self-procured, and the employee will be responsible for 100% of the cost, with the exception of employees who report to the Emergency Room for urgent cases.
